Source text - Spanish Bienvenidos al primer curso e-learning de Humira en psoriasis. El objetivo será reforzar y ampliar el nivel de conocimientos alcanzado en el programa de formación inicial.
Para ello, hemos dividido este curso en dos módulos.
En el primero de ellos se tratarán las características básicas de Humira, como son el desarrollo del producto, el mecanismo de acción y una revisión de su perfil de eficacia y seguridad.
En el segundo módulo hablaremos de los principales ensayos clínicos que se han realizado con Humira en Psoriasis, trabajando de una manera muy práctica los estudios CHAMPION y REVEAL.
Finalmente realizarás un cuestionario con el fin de que compruebes si has alcanzado el nivel de conocimientos requerido, y lo compares con la prueba que ya has realizado.
Al finalizar este primer módulo de aprendizaje serás capaz de:
Entender, para poder explicar a tus clientes cómo se consiguió que Humira fuera el primer anticuerpo monoclonal totalmente humano y qué implicaciones tiene esto en los pacientes.

En tercer lugar, profundizarás en el perfil de eficacia y seguridad global de Humira, desde su primera indicación en artritis reumatoide hasta nuestros días. De este modo serás capaz de transmitir al médico que Humira es el biológico más completo para sus pacientes .
Por último, conocerás todos los detalles sobre la nueva forma de adminisitración de Humira: la PLUMA. Este mecanismo se ha diseñado para facilitar la vida de los enfermos.
Humira, conocido al principio con el nombre de D2E7, fue descubierto en el centro ABC de Boston en 1995.
En 1997 fue estudiado por primera vez en humanos. En 2000 comenzaron los estudios Fase III en pacientes con Artritis Reumatoide, y los primeros resultados se recogieron a mediados de 2001. A principios de 2002 se presentaron los datos a la FDA, y a la EMEA, y fue aprobado por la FDA el 31 de diciembre de ese año.
En España se lanzó Humira en marzo del 2004 con la indicación de Artritis Reumatoide.
En Septiembre de 2005 llegó la indicación de Artritis Psoriásica. En junio de 2006 se lanzó Espondilitis Anquilosante y por último en octubre de 2007 la Enfermedad de Crohn.
El primer paso en el desarrollo de Humira se denomina Técnica de Presentación de fagos, y consiste en mezclar anticuerpos de diferentes donantes humanos para conseguir gran cantidad de combinaciones con afinidades a diferentes antígenos. De este modo, obtenemos la llamada “Biblioteca de anticuerpos humanos”.
Una vez que tenemos todas estas combinaciones, el siguiente paso será comprobar su afinidad por el TNF-alfa, (aparece foto 2) haciéndolas pasar por un tubo cuyas paredes internas están impregnadas de esta proteina. A esto se le llama una Columna de Afinidad.
Con este proceso somos capaces de recoger las combinaciones HUMANAS más afines al TNF-alfa aunque no son del todo específicas. Esto constituye un avance biotecnológico muy importante. Para conseguir el anticuerpo perfecto, la mejor opción es introducir el TNF-alfa HUMANO en un animal.
Existen en el mercado anticuerpos anti-TNF procedentes 100% de ratón, como es el caso del producto SEGARD®.
Estos anticuerpos son perfectos en cuanto a afinidad por el TNF-alfa humano, debido a que el organismo del ratón no reconoce esta proteina como propia, y la rechaza fabricando estos anticuerpos. Aunque la afinidad es excelente, ocasionan grandes problemas de seguridad, y de eficacia a largo plazo, cuando se emplean en tratamientos crónicos debido a su origen animal (de ratón) y no humano
Lo ideal sería, por tanto, conseguir la afinidad perfecta de este anticuerpo murino, y la eficacia y seguridad de un anticuerpo humano, lo cual nos lleva al 2º paso
Translation - English Welcome to the first Humira e-learning course on psoriasis. The objective will be to reinforce and broaden the knowledge level reached in the initial training programme.
To do that, we have divided this course into two modules.
In the first module, the basic characteristics of Humira will be addressed, such as product development, the mechanism of action and a review of its efficacy and safety profile.
In the second module, we will use the CHAMPION & REVEAL studies to deal in a practical way with the main clinical trials that have been carried out with Humira on psoriasis.
Finally, you will complete a questionnaire to test if you have reached the required knowledge level and of comparing it with the test previously undertaken.
On completion of this first learning module you will able to:
Understand and be able to explain to your clients how Humira became the first fully human monoclonal antibody and the implications that this has for the patient.
We will analyse, as you began to see in the initial training course, Humira’s mechanism of action as an anti-TNF and the impact that this has on the disease psoriasis.
Thirdly, you will study in depth Humira’s global efficacy and safety profile, from its first use in rheumatoid arthritis to the present-day. You will thus be able to inform doctors that Humira is the most complete biologic for his/her patients.
Finally, you will get to know all the details about the new method for administering Humira: the PEN, a mechanism that has been designed to make patients’ lives easier.
Humira, initially known as D2E7, was discovered in 1995 in the ABC Centre in Boston. It was studied for the first time in humans in 1997. Phase III studies began in 2000 in patients with rheumatoid arthritis and the first results were collected in the middle of 2001. At the start of 2002 the data was presented to the FDA and the EMEA, and it was approved by the FDA on the 31 December of that year.
Humira was launched in Spain in March 2004 for use in rheumatoid arthritis.
In September 2005 it received the indication for psoriatic arthritis. In June 2006 it was launched for ankylosing spondylitis and, finally, in October 2007 for Crohn’s disease.
The first step in the development of Humira is phage display technology which consists of mixing antibodies from different human donors to obtain a large number of combinations that have affinities to different antigens. In this way, we obtain the so-called ‘Human Antibody Library’.
Once we have these combinations, the next step is to confirm their affinity to TNF-alpha, (photo 2 appears) passing them through a tube whose internal walls are impregnated with this protein. This is called an affinity column.
Using this process we are able to gather the HUMAN combinations that are most similar to TNF-alpha even though they are not completely specific. This constitutes a very important biotechnological advance. To get the perfect antibody, the best option is to introduce HUMAN TNF-alpha into an animal.
Anti-TNF antibodies originating 100% from mice exist in the market, such as SEGARD®.
These antibodies are perfect in terms of their affinity to human TNF-alpha, due to the fact that the mouse’s organism does not recognise this protein as its own and rejects it by making the antibodies. Despite their excellent affinity, serious problems of safety and long-term efficacy arise when they are used in chronic treatments owing to their animal (from mice) and not human origins.
The ideal would be, therefore, to achieve the perfect affinity of this rodent antibody with the efficacy and safety of a human antibody, which takes us to the 2nd step.

La expresión con la que titulamos es muy usada en Italia para indi-car la presencia de algo que, si bien es útil, está muy lejos de ser necesario. Se nos ocurrió inmedia-tamente teniendo en cuenta la ex-cesiva difusión de ferias dedica-das al fenómeno de las private la-bel. No caben dudas que se trata de un fenómeno importante y en plena expansión, pero nos parece que alrededor de veinte ferias es-pecializadas (si bien distribuidas en la misma cantidad de países) son demasiadas.
No podemos dejar de recordar que cuando (hace 20 años) crea-mos PLE el más benévolo de los comentarios fue que éramos “vi-sionarios” o un poco ilusos: ahora todos compiten por imitarnos o por lo menos entrar en este mercado.
Lo mismo ocurre con las ferias, como lo demuestra el hecho que justamente en estos días abrió sus puertas la feria de las private label de Munich. Está bien que Alema-nia es uno de los mercados más beneficioso para las private label, pero no nos olvidemos el gran flor de una exposición similar que se llevó a cabo en Colonia hace aproximadamente una década.
El mismo comentario se puede hacer para la Marca de Bologna que fue presentada como la “pri-mera manifestación” italiana del sector, olvidando que en el pasa-do se registraron dos grandes fracasos en (MAR.CO e Mediterra-nean Private Label) y otro ubicado en los alrededores de la ciudad bo-loñesa “Italian Private Label”). Que la actual feria se está manifes-tando como un humilde éxito es una realidad, pero se trata siem-pre de un éxito limitado si se compara con las dos grandes manifestaciones organizadas por PLMA en Ámsterdam y Chicago, que elaboraron (y siguen hacién-dolo), el gran éxito mundial de las private label.
Actualmente con la new wntry de Munich y además de la ya men-cionada Marca di Bologna y a las dos “grandes”de PLMA, se orga-nizan similares manifestaciones en Dubai, Istambul, Barcelona, París, New York, Tokio, Shangai e Mosú y se anuncian las de Lon-dres y de Milán (Tuttofood que si bien no está dedicada a las priva-te label, se presenta especial-mente abierta en ese sentido) y el SIAL de París que abrió un sector especial dedicado específicamen-te a las private label.
Es más, se comenta que otras iniciativas similares se están por abrir en Sudamérica.
Si la empresa productora quiere visitar durante el año una de es-tas muestras se encontraría con costos insostenibles. ¿Nadie piensa en esto?
Cada uno piensa en cuidar su quintita rodeado de pocos cente-nares de expositores, pero el mercado queda descolocado.
Si una cadena de distribución quisiera sólo visitar todas las muestras que detallamos debería tomar personal especializado es-pecialmente dedicado a este fin. Sin obtener ningún beneficio.

The above expression is often used in Italy to indicate the presence of something that, even though it is useful, is far from necessary. It immediately occurred to us when bearing in mind the excessive spread of trade fairs dedicated to the private label phenomenon. There is no doubt that we are dealing with a significant and growing phenomenon but, to us, 20 specialised trade fairs (even though they are distributed in the same number of countries) is just too many.
It is impossible to forget that when we created PLE 20 years ago the most benevolent comment was that we were ‘visionaries’ or a little naive: now, everyone is competing to emulate us or to, at least, enter the market.
The same thing happened with trade fairs, as shown by the fact that in last few days the Munich private label trade fair opened its doors. Germany may be one of the most profitable markets for private labels but let’s not forget the great flop of a similar exhibition that was held in Cologne about a decade ago.
The same can be said about the Marca (Bologna) fair that was presented as the sector´s ‘first Italian event’, forgetting that in the past, three great failures were re-corded in (MAR.CO and Mediterranean Private Label) or around (Italian Private Label) the Bolognese city. That the current trade fair is proving to be a modest success is true, but its success will always be limited when compared to the great shows organised by PLMA in Amsterdam and Chicago, which created and continue to create private labels’ global success.
Currently , with the recent entry of Munich and of the aforementioned Marca (Bologna) and the two PMLA ‘big ones’, similar events are being organised in Dubai, Istanbul, Barcelona, Paris, New York, Tokyo, Shanghai and Moscow, and have been announced for London and Milan (Tuttofood, that even though is not dedicated to private labels is especially open in that respect) and the SIAL in Paris (that opened a sector specifically dedicated to private labels).
And that’s not all. It is said that similar initiatives are about to open in South America.
If a production company wanted to visit one of these shows in the course of the year, it would be faced with unsustainable costs. Hasn’t anyone thought of that?
Everyone focuses on looking after their own patch, surrounded by a few hundred exhibitors, but the market remains a mess.
If a distribution chain wanted to visit all the shows that we have mentioned it would have to employ specialised staff solely for this purpose; and, for no benefit.

INTRODUCCIÓN
El objeto de ésta especificación es la definición del sistema de
acondicionamiento de aire para pasajeros del coche doble piso EMFER.
La solución utilizada consiste en instalar dos equipos compactos por Tren,
diseñados para montaje sobre el techo.
Estos equipos de climatización tienen dos compresores herméticos, con
refrigerante R407C, o sea dos circuitos frigoríficos que permiten una regulación
de capacidad del equipo (50% - 100%).
El panel de control, que contiene la electrónica de control (contactores,
interruptores, automáticos, etc.) se instalará en el interior de cada equipo de
Aire Acondicionado.
Todas las funciones del Sistema de Acondicionamiento de Aire serán
gobernadas por un Módulo de Control que incorpora la más avanzada
tecnología en el campo de la Regulación Automática, mediante el empleo de un
microprocesador como elemento principal. Este módulo se instalará en el panel
de control dentro del equipo de aire acondicionado.
El convertidor de energía eléctrica a partir de la fuente de 800Vcc, estará
integrado en cada uno de los equipos compactos y será objeto de una
especificación aparte.
PRESTACIONES NECESARIAS
Las prestaciones proporcionadas por dichos equipos se han previsto en base a
los siguientes datos:

5. DESCRIPCIÓN DEL EQUIPO
El equipo de Aire Acondicionado se montará sobre cada uno de los vestíbulos
extremos.
3.1. Prestaciones en funcionamiento normal
Los dos equipos compactos tendrán las características técnicas siguientes:

3.2. Conductos de Distribución
La distribución de aire en el coche se realizará por medio de un conducto
central que dirige el aire desde el equipo hasta los conductos situados en cada
unos de los pisos: superior e inferior. Los conductos estarán adecuadamente
aislados para evitar condensaciones de agua en los mismos.
La máxima velocidad del aire dentro de dichos conductos durante calefacción y
refrigeración es menor de 6 m/s.
3.3. Control de Temperatura
El control electrónico de temperatura por microprocesador, estará centralizado
en un armario dentro del equipo de aire acondicionado.
El panel de fuerza (contactores, automáticos, relés térmicos, etc.) está ubicado
dentro del equipo de Aire Acondicionado.

5. DISTRIBUCIÓN DE AIRE
La distribución de aire dentro del vehículo se realiza de la siguiente manera:
5.1. Aire Viciado
Saldrá al exterior del vehículo por medios provistos por el carrocero.
5.2. Aire Exterior
El aire fresco será aspirado directamente por las rejillas laterales en cada
grupo, y mezclado con el aire de retorno.
El caudal de aire exterior por equipo es de de 1375 m3/h ±10%.
5.3. Aire de Retorno
Cada equipo dispondrá de rejillas de admisión del aire de retorno en la zona
de inferior.
El caudal de aire de retorno es de aproximadamente de 2.725 m3/h por
equipo de A.A.
5.4. Aire Tratado
El aire impulsado por cada equipo será de 4100 m3/h.
5.5. Niveles máximos de Ruido Interior:

6. CONTROL
El funcionamiento del equipo estará controlado por un microprocesador que
controlará cada uno de los modos de funcionamiento que proporciona el
mismo, a saber: Pre-calefac., Pre-refrigeración, Ventilación, Refrigeración,
Calefacción, etc.
Una vez que el control obtenga las medidas de temperaturas, dará las ordenes
necesarias para activar los elementos requeridos en cada caso específico, con
el fin de obtener - dentro del coche y en un tiempo dado - la temperatura
requerida en la especificación.
El control también incluirá la detección de máxima/mínima tensión, desfase de
arranque de compresor, etc.
Los tiempos de funcionamiento del compresor y motores ventiladores, así como
las temperaturas principales, quedarán memorizadas, para facilitar el
mantenimiento del equipo y para verificar su funcionamiento.
Los datos que se mencionan más arriba se podrán leer por medio de un
computador tipo "laptop" y podrán ser procesados para obtener datos para el
mantenimiento preventivo adecuado.
El control del HVAC recibe del coche las siguientes señales:
- señal marcha / paro
- tensión de batería 100 Vdc
así mismo el control envía al coche las siguientes señales:
- fallo ventilación
- fallo calor / frío
- fallo convertidor.

INTRODUCTION
The aim of this specification is the definition of the air conditioning system for passengers of the EMFER double-decker carriage.
The solution used consists of the installation of two compact units per train, designed to be mounted on the roof.
These air conditioning units have two hermetic compressors containing R407C coolant, i.e. two cooling circuits that allow for regulation of the system’s capacity (50% -100%).
The control panel containing the control electronics (contactors, switches, circuit breakers, etc.) is installed inside each air conditioning unit.
The system’s functions are controlled through a control module that incorporates the most advanced technology in the field of automatic control, its principal element being a microprocessor. This module in installed in the control panel within the air conditioning unit.
The electrical energy converter from the 800VCC source is integrated into each of the compact units and is dealt with in a separate specification.

According to the previous data the forecast required calorific power is 36,000 W per train.
The heating power is obtained through 36kW heater elements installed in each carriage (2 units).
These elements have a power capacity of 18 kW per unit, supplied by a 360VAC power source.
5. UNIT DESCRIPTION
Air conditioning units are assembled on each of the end vestibules.
3.1. Performance during normal operation
The two compact units have the following technical characteristics:

3.2. Distribution Ducts
The distribution of air in the carriage is carried out through a central duct that directs air from the unit to ducts situated on each level of the carriage: upper and lower. To avoid water condensation within the ducts they are suitably insulated.
The maximum air speed within the ducts during heating and cooling is less than 6 m/s.
3.3. Temperature control
The electronic temperature microprocessor control is centralised in a cabinet within the air conditioning unit. The control panel (contactors, circuit breakers, thermic relays, etc.) is located inside the air conditioning unit.

The return air flow is approximately 2,725 m³/h per air conditioning unit.
5.4. Treated air
The air propelled by each unit is around 4,100 m³/h.

6. CONTROL
Unit operation is regulated by a microprocessor that controls each of the its operation modes, namely: Pre-heating, Pre-cooling, Ventilation, Cooling,
Heating, etc.
Once the control obtains the temperature measurements it gives the necessary orders to activate the elements required in each specific case with the aim of reaching – inside the carriage and in a given time – the temperature stated in the specification.
The control also includes the detection of the maximum/minimum voltage, the time lag in the start of the compressor etc.
The operating times of the compressor and fan motors, as well as the main temperatures are memorised to facilitate unit maintenance and for operation checks.
The aforementioned data can be read by a laptop and can be processed to obtain data for appropriate preventative maintenance.
The HVAC control receives the following signals from the carriage:
- start/stop signal
- battery voltage 100 VDC
In addition, the control sends the carriage the following signals:
- ventilation failure
- hot/cold failure
- converter failure
