Source text - English Biotic Degradation: Product® is considered to be readily biodegradable. Product® was tested under Test Guideline OECD 310 and the results showed the mean production of CO2 by mixtures containing Product® was equivalent to 10% of the theoretical maximum after approximately Day 1, and 61.6% Day 10. Substances are considered to be readily biodegradable in this test if CO2 production is equal to or greater than 60% of the theoretical value within ten days of the level achieving 10%. Product® can, therefore, be considered as readily biodegradable.
An earlier study used a static closed bottle test with no measures to enhance the bioavailability of the test substance. Results indicated that biodegradation of Product® was slow and achieved, at most, 57% in a 28-day period and did not attain the criteria for ready biodegradation. As Product® is a poorly soluble low density liquid, the sealed-vessel carbon dioxide evolution test was considered a more appropriate test system to assess its ready biodegradability.
Activated Sludge Respiration Inhibition: Product® had no inhibitory effect on the respiration rate of activated sewage following three hours of exposure at 1, 10, and 100 mg/l. Therefore, the three-hour EC50 could not be calculated but must be greater than 100 mg/l, the highest level tested.
Hydrolysis: The hydrolysis of Product® as a function of pH was studied according to EEC guidelines. The preliminary study showed that at pH 4, 7, 9 and 50 C, greater than 10% hydrolysis had occurred after 5 days, equivalent to an environmental half-life at each pH value of less than 1 year. Product® is hydrolytically unstable under acidic, neutral and basic conditions.

Of course, without knowing more about each of these patients, it may be difficult to make a decision.
Jeremy is 52 years old, was diagnosed with Type 2 diabetes 1 year ago, and is uncontrolled on metformin with an HbA1c of 9%. I'd like you to consider the product XX for this patient when he is not at his treatment goal on metformin alone.
Looking to the next patient. . .Mary is 49 years old, was diagnosed with Type 2 diabetes 2 years ago,
has an HbA1c of 8% even though she is currently taking metformin, and she has concerns about hypoglycemia. For Mary, I'd like you to consider the product YY as an effective and flexible add-on therapy.
I'd like to show you why the product YY & the product XX are good options for these and other patients as shown in clinical studies with sitagliptin.

Improved glucose control with the product YY & the product XX has been shown in several clinical studies – one of which showed a 2.1% reduction in HbA1c in patients (with a baseline of 8.8%) taking sitagliptin plus metformin versus a 1.3% reduction in patients taking metformin alone. Patients with a higher baseline also got a powerful 2.9% reduction after 24 weeks.

Importantly, these reductions were sustained over time. Not only did more patients achieve HbA1c goal, more patients stayed at goal. As you can see, 90% of patients who had achieved a significant reduction after 24 weeks maintained that reduction after 54 weeks.
In fact, 66% – nearly twice – as many patients achieved goal at 24 weeks. Other important efficacy results from this study were the improved measures of β-cell function, including significant improvement in HOMA-β, which is a measure of the ability of pancreatic β-cells to secrete insulin under fasting conditions.

Dans plusieurs essais cliniques, on a observé avec le produit YY et le produit XX une amélioration du contrôle glycémique – un de ces essais mettait en évidence une diminution d'HbA1c de 2,1 % chez les patients (pour une valeur initiale de 8,8 %) prenant de la sitagliptine associée à la metformine comparativement à une diminution de 1,3% chez les patients prenant de la metformine seule. Les patients ayant une valeur initiale plus élevée présentait également une diminution forte de 2,9 % après 24 semaines.

Il est important de noter que ces diminutions étaient maintenues dans le temps. Non seulement davantage de patients atteignaient le taux d'HbA1c visé, mais aussi davantage de patients conservaient ce taux cible. Comme vous pouvez le constater, 90 % des patients qui étaient parvenus à une diminution significative après 24 semaines conservaient cette diminution après 54 semaines.
En fait, 66 % - presque le double – des patients atteignaient l'objectif au bout de 24 semaines. D'autres résultats importants relatifs à l'efficacité d'après cette étude étaient l'amélioration des marqueurs de la fonction des cellules β, notamment une amélioration significative de l'indice HOMA-β, qui est une mesure de la capacité des cellules β pancréatiques à sécréter de l'insuline dans des conditions de jeûne.
