Вообще-то идиома - это средство художественной выразительности и для перевода "текста медицинской тематики" (впрочем, как и любого другого специального текста) принципиального значения не имеет. Поэтому логичнее перевести по сути, тем более что иначе есть риск быть просто неправильно понятым.
